Eskaton Collaborates with Sun City Residents to Encourage Aging in Place

“Aging in place” has become one of the most prominent trends to emerge since the baby-boom generation began the migration into retirement. Eskaton, one of Northern California’s preeminent senior residential and services providers has begun a visionary collaboration with residents in Sun City Lincoln Hills to prove out how their partnership will contribute to success in navigating later life transitions.

A group of forward-thinking residents in Sun City Lincoln Hills reached out to local experts to determine the best way to meet the information and service needs of their residents in addition to older adults living in the vicinity. After many months of planning, research and discussion, Resource Connectors was born.

Resource Connectors is a collaborative effort of The Lincoln Hills Foundation, Live Well at Home by Eskaton, Right at Home Senior Care and Sutter Medical. As residents encounter health problems and issues that challenge their continued independence, Resource Coordinators are available FREE by phone to provide immediate connections to community resources and screened service providers.

In addition to their 24/7 information support, Resource Connectors provides free Home Safety Assessments and fee-for-service professional care management as well as in-home care services.  Lincoln Hills residents may request an in-home visit with a Resource Coordinator in to discuss their individual concerns. While there, the Resource Connectors staff will evaluate the household and offer advice or suggestions particular to meeting individual safety concerns of the resident.
